These guys are villains that genuinely disgust and terrify you. They're not like the Joker, Davros, the Master etc, who may disgust and terrify you, but are charismatic, enjoyable to watch and at certain points, funny in a sick way.
These guys truly are the stuff of nightmares.
I'm going to say
The Coachman from Pinocchio.
This guy terrified me so much growing up. He is a Demon that lures children away to "Pleasure Island" where he plies them with spiked drinks that eventually turn them into Donkeys. Worse they are still aware when they are Donkeys, but can't talk. He then sells them for skin, meat, labour etc. The scene where one of the poor boys begs to go back to his mother, only for this monster to laugh at his misfortune scared and saddened me so much as a boy I didn't watch the rest of the film, and for years had no idea there was even a whale in Pinocchio LOL.
Here he is in action.
You know the scariest thing about him? He gets completely away with it! The little boys are never rescued and he is never held accountable for his crimes. He's still out there!
Honestly this guy is like if Pennywise and the Grand High Witch f*cked and had a baby. (I so wish there had been a live action Pinocchio in the 70s with Peter Cushing as Pinocchio's dad, Ray Harryhausen doing the special effects for Monstro and Tim Curry as the Coachman. Imagine how horrifying Curry would have been as that character? Might have been too scary to watch.)
The Grand High Witch is up there too. I've talked about this in the past so I won't get into that again. Also I fancied her too LOL (in her human form obvs.) She had a big effect on me that way as thanks to her I seem to like dominant women, with jet black hair and HUGE noses. Still kinkiness aside she is still an absolutely horrifying character. That bit with Erica in the painting is as terrifying as the Coachman's treatment of the Donkey kids.
Finally another villain that perhaps worked a little too well, was the Gnarl. A Demon from Buffy that paralyses its victims, and then peels their skin off when they are alive, strip by strip.
I was lucky that I was a bit older when I saw this, as if I'd seen this as a child I never would have watched Buffy ever again! Even then though he still managed to give me nightmares.
